New ICD-10-CM Codes for Partially & Unvaccinated Patients

You’ll soon have new ICD-10-CM Z codes to report when treating patients who are partially vaccinated or unvaccinated for COVID-19. Unvaccinated patients.
This new code set, effective April 1, 2022, includes four additions under Chapter 21 (Factors influencing health status and contact with health services). The codes were added to better track those who aren’t fully vaccinated and who require medical attention.

The added Z codes are:

  • New subcategory Z28.31: Underimmunized for COVID-19 status
  • Z28.310: Unvaccinated for COVID-19
  • Z28.311: Partially vaccinated for COVID-19
  • Z29.39: Other underimmunized status

Use code Z28.310 for patients who have not received at least one dose of any COVID-19 vaccine. For those patients who have received at least one dose of a multi-dose regimen (such as Pfizer or Moderna) but have not received the full set of doses required to meet the CDC definition of “fully vaccinated,” use code Z28.311.

Patients who are delayed or lapsed in getting other non-COVID vaccines are considered “underimmunized” and you should report code Z29.39. Unvaccinated patients.

These codes are not to be reported for patients who aren’t eligible to receive a COVID-19 vaccine.
