When it comes to provider onboarding, time really is money at your medical practice. For every day that a new provider’s credentialing is delayed you miss a full day of being able to bill Medicare for their services, which can be expensive.

One tried and true way of helping move your provider onboarding along faster is by creating a checklist.

Provider Onboarding Checklist

Print out the following checklist and share it with your new clinicians during provider onboarding to ensure that you’re able to complete their credentialing and enrollment faster.

  • Full legal name (include former names or maiden name)
  • Home address
  • Email address
  • Cell phone number
  • Specialty
  • NPI number (if applicable)
  • CAQH login details (if applicable)*
  • Expected start date
  • Expected date to start seeing patients
  • Primary practice location
  • Current resume — all work experience and education MUST have start and end dates
  • Malpractice certificate of insurance (including effective date and coverage limits)
  • Malpractice history
  • Copy of medical/nursing/dental degree(s) (if applicable)
  • Copy of all training, internship, residency, fellowship certificate(s) (if applicable)
  • Copy of all board certifications
  • Copy of diplomas/certificates from all degrees/programs (if applicable)
  • Copy of state licenses
  • Copy of DEA and controlled substance certificate (if applicable)
  • Copy of Social Security card
  • Copy of driver’s license
  • Copy of ECFMG certificate (if applicable)
  • Copy of national practitioner data bank self-query
  • Three professional references
  • Hospital affiliations/admitting arrangements
  • Supervising/collaboration physician (if PA or NP)
  • Medicare or Medicaid number (if applicable)
  • Date of birth
  • Place of birth
  • Social Security number
  • Languages spoken
  • If not a US citizen, proof of right to work in the US
  • Full explanation of any disciplinary action, misdemeanors, felonies, etc.

* If the provider already has an active CAQH profile, the practice can obtain most of the remaining information on this form from the profile.

Once you get the above information from the new provider, you should be able to significantly cut your provider onboarding time. Having these details on-hand before you begin will allow you to avoid having to go back to the provider repeatedly for additional information.
