What Modifier 25 Means (And Why It Matters for Your Practice)

Modifier 25 tells payers you provided a significant, separately identifiable Evaluation & Management (E/M) service on the same day as a minor procedure. According to CMS and CPT® guidance, the evaluation normally required before a minor procedure is already included in the procedure payment, so you must clearly document additional medical necessity to bill separately.

CMS explains this in the National Correct Coding Initiative (NCCI) manual: If your documentation doesn’t clearly show separate work, claims may be denied, downcoded, or audited. These principles were emphasized in your training source material as foundational for correct Modifier 25 usage.

When You SHOULD Use Modifier 25

You should use Modifier 25 when the patient needs care beyond routine procedural evaluation. This may include evaluating a new complaint, managing an exacerbation of a chronic condition, or addressing additional symptoms that require medical decision-making separate from the procedure.

The American Medical Association CPT® guidance explains that the E/M service must be “significant and separately identifiable.”

For example, if a patient comes in for a scheduled injection but also requires evaluation of worsening hypertension, medication adjustments, or diagnostic decisions, Modifier 25 may be appropriate when clearly documented.

When Modifier 25 Should NOT Be Used

You should not append Modifier 25 simply because a visit occurred on the same day as a procedure. If the evaluation was only to determine whether to perform the procedure or to carry it out, that work is bundled into the procedure reimbursement.

The Office of Inspector General (OIG) has historically identified improper Modifier 25 billing due to insufficient documentation. Avoid “defensive coding” where the modifier is added automatically. Unsupported use increases denial risk, payer scrutiny, and potential recoupments.

CMS defines global surgical packages that bundle pre-operative, intra-operative, and post-operative services into one payment. Minor procedures usually have 0- or 10-day global periods, while major surgeries often have 90-day global periods.
